Roster, invites, and active plans in one working surface.

Create the patient, issue the invite, and publish the one active plan they see first in the iPhone app. This screen is optimized for that handoff, not generic dashboard chrome.

This provider surface exchanges the upstream OAuth identity token for a Concky API session, so the same backend can be used from the provider dashboard and the iPhone app.

Apple web sign-in remains disabled until the Apple provider is configured separately.
